We do\r\n not share your health information with anyone that does not require it, but we do need to\r\n know about any potential health issue or medication, so we can help you if needed.\r\n <\/p>\r\n <p>\r\n Volunteers will be traveling to remote areas with limited access to medical care and\r\n medications. If you require medications, please be sure to pack them with you! If\r\n possible, carry a refill prescription separately, we cannot guarantee a suitable\r\n substitute will be available.\r\n <\/p>\r\n <p>\r\n Expect your trip to be physically and emotionally demanding both as a result\r\n of activities and living conditions. During our trips you may experience\r\n significant climate and weather changes, strenuous physical activity, loss\r\n of sleep, and an unfamiliar diet. Only you and your physician can fairly\r\n evaluate your fitness for this kind of activity.\r\n <\/p>\r\n <p>\r\n Your trip will require preparation which may include immunizations,\r\n other shots or treatments, or other preparation. Please consult your\r\n physician prior to your trip with adequate time to meet those\r\n demands.\r\n <\/p>\r\n <p>\r\n Our employees are not doctors or medical experts. It is your\r\n responsibility to plan and provide for your own health and well\r\n being for the duration of your trip. If you have a potential\r\n health issue or cause for our concern, let us know before your\r\n trip what you require. This Agreement\r\n and Release is entered into in\r\n connection with the trip offered by BTC\r\n (\u201cTrip\u201d) under which BTC provides the\r\n opportunity for Volunteer to participate\r\n in volunteer service and cultural\r\n exchanges. Volunteer enters into this\r\n Agreement and Release in consideration\r\n of the opportunity to participate in the\r\n trip and for other good and valuable\r\n consideration, the sufficiency of which\r\n the parties acknowledge.\r\n <\/p>\r\n <p>\r\n If Volunteer is younger than 18 years\r\n of age, this Agreement and Release\r\n must be signed by a parent or legal\r\n guardian of Volunteer, and, in such\r\n case, all references below to\r\n Volunteer include his or her parents\r\n or guardian. This document is a\r\n legally binding release, which, in\r\n certain situations, will reduce or\r\n eliminate Volunteer's legal rights\r\n and legal recourse. We accept Visa\r\n ,Mastercard, and American\r\n Express. We will also accept\r\n checks made payable to Bridges\r\n to Community,Inc. All payments\r\n must indicate the name of the\r\n project or volunteer for whom\r\n they're intended, as well as the\r\n dates of the trip.\r\n <\/p>\r\n \r\n <h4>Project Cost<\/h4>\r\n <p>The project cost must be\r\n paid in full no later\r\n than six (6) weeks prior\r\n to the date the group is\r\n set to travel.\r\n <\/p>\r\n <p>\r\n BTC will only guarantee\r\n your trip and itinerary\r\n if they project cost has\r\n been paid in full.\r\n Failure to pay the\r\n project cost will result\r\n in BTC cancelling your\r\n trip. All donations and\r\n payments made toward the\r\n project cost and\r\n received by BTC are\r\n nonrefundable.\r\n <\/p>\r\n \r\n <p>\r\n In the event that a\r\n group does not fully\r\n pay for the project\r\n before the scheduled\r\n trip date, BTC will\r\n hold the project\r\n funds to be used for\r\n a future trip within\r\n one (1) calendar\r\n year. If the group\r\n does not reschedule\r\n a trip within that\r\n time period, BTC\r\n reserves the right\r\n to use the funds\r\n received to complete\r\n the project and\/or\r\n apply the funds to\r\n our general\r\n programming.\r\n <\/p>\r\n <h4>Volunteer Trip Fee<\/h4>\r\n \r\n <p>\r\n The\r\n completed\r\n Volunteer\r\n Application\r\n and the\r\n balance\r\n of your\r\n volunteer\r\n trip fee\r\n must be\r\n submitted\r\n no later\r\n than six\r\n (6)\r\n weeks\r\n prior to\r\n the\r\n start of\r\n your\r\n trip.\r\n <\/p>\r\n <h4>Trip Cancellation by BTC<\/h4>\r\n \r\n <p>At\r\n any\r\n time\r\n prior\r\n to\r\n the\r\n Program\r\n start\r\n date,\r\n BTC\r\n may,\r\n at\r\n its\r\n sole\r\n discretion,\r\n cancel\r\n a\r\n Volunteer's\r\n registration,\r\n or\r\n cancel\r\n the\r\n trip\r\n in\r\n which\r\n Volunteer\r\n is\r\n scheduled\r\n to\r\n participate.\r\n In\r\n the\r\n unlikely\r\n event\r\n of\r\n such\r\n a\r\n cancellation,\r\n trip\r\n fees\r\n paid\r\n to\r\n BTC\r\n by\r\n Volunteer\r\n for\r\n that\r\n trip\r\n may\r\n be\r\n refunded.\r\n In\r\n the\r\n unlikely\r\n event\r\n of\r\n such\r\n a\r\n cancellation,\r\n BTC\r\n will\r\n hold\r\n project\r\n related\r\n funds\r\n and\r\n attempt\r\n to\r\n reschedule\r\n the\r\n volunteer\r\n group\r\n within\r\n one\r\n calendar\r\n year.\r\n <\/p>\r\n <p>In\r\n the\r\n event\r\n of\r\n a\r\n cancellation\r\n by\r\n BTC\r\n due\r\n to\r\n any\r\n extraordinary\r\n event\r\n or\r\n circumstance\r\n beyond\r\n the\r\n control\r\n of\r\n BTC,\r\n such\r\n as\r\n a\r\n war,\r\n strike,\r\n riot,\r\n act\r\n of\r\n terrorism,\r\n or\r\n an\r\n event\r\n described\r\n by\r\n the\r\n legal\r\n term\r\n \"Act\r\n of\r\n God\"\r\n (such\r\n as\r\n hurricane,\r\n flooding,\r\n earthquake,\r\n volcanic\r\n eruption,etc.),\r\n beyond\r\n the\r\n control\r\n of\r\n BTC\r\n that,\r\n in\r\n the\r\n sole\r\n opinion\r\n of\r\n BTC,\r\n renders\r\n travel\r\n or\r\n participation\r\n in\r\n a\r\n trip\r\n unsafe\r\n or\r\n impracticable,\r\n trip\r\n fees\r\n will\r\n be\r\n refunded\r\n with\r\n the\r\n exception\r\n of\r\n the\r\n $250\r\n deposit\r\n and\r\n BTC\r\n will\r\n hold\r\n project\r\n related\r\n funds\r\n and\r\n attempt\r\n to\r\n reschedule\r\n the\r\n volunteer\r\n group\r\n within\r\n one\r\n calendar\r\n year.\r\n In\r\n those\r\n and\r\n all\r\n other\r\n circumstances,\r\n BTC\r\n shall\r\n not\r\n be\r\n responsible\r\n to\r\n Volunteer\r\n for\r\n any\r\n other\r\n refund,\r\n costs,\r\n interest,\r\n liability,\r\n losses,\r\n or\r\n damages\r\n of\r\n any\r\n kind,\r\n including\r\n airfare.\r\n <\/p>\r\n <h4>Withdrawal by Volunteer<\/h4>\r\n \r\n <p>\r\n If\r\n Volunteer\r\n withdraws\r\n for\r\n any\r\n reason,\r\n Volunteer\r\n must\r\n notify\r\n BTC\r\n in\r\n writing.\r\n If\r\n Volunteer\r\n voluntarily\r\n withdraws\r\n from\r\n the\r\n trip,\r\n funds\r\n paid\r\n by\r\n Volunteer\r\n to\r\n BTC\r\n will\r\n be\r\n refunded\r\n only\r\n as\r\n set\r\n forth\r\n below:\r\n <\/p>\r\n <p>A trip\r\n fee\r\n deposit\r\n of\r\n $500\r\n deposit\r\n will\r\n not\r\n be\r\n refunded.\r\n <\/p>\r\n <p>\r\n Any\r\n further\r\n trip\r\n Fee\r\n payments\r\n made\r\n may\r\n be\r\n refunded\r\n or\r\n held\r\n for\r\n a\r\n future\r\n trip\r\n within\r\n one\r\n (1)\r\n calendar\r\n year.\r\n <\/p>\r\n <p>No\r\n portion\r\n of\r\n any\r\n Trip\r\n Fee\r\n paid\r\n shall\r\n be\r\n refunded\r\n if\r\n the\r\n withdrawal\r\n is\r\n received\r\n one\r\n week\r\n before\r\n the\r\n trip\r\n start\r\n date.\r\n <\/p>\r\n <p>\r\n Changes\r\n to\r\n and\r\n cancellation\r\n of\r\n airfares\r\n are\r\n subject\r\n to\r\n the\r\n regulations\r\n of\r\n the\r\n providers,\r\n and\r\n any\r\n travel\r\n fees,\r\n charges,\r\n expenses\r\n or\r\n penalties\r\n that\r\n might\r\n result\r\n from\r\n any\r\n cancellation\r\n of\r\n or\r\n withdrawal\r\n from\r\n the\r\n trip\r\n will\r\n be\r\n the\r\n sole\r\n responsibility\r\n of\r\n the\r\n Volunteer.\r\n <\/p>\r\n <h4>Alterations to Program<\/h4>\r\n \r\n <p>\r\n BTC\r\n reserves\r\n the\r\n right\r\n to\r\n change\r\n the\r\n trip\r\n due\r\n to\r\n severe\r\n weather,\r\n acts\r\n of\r\n war\r\n or\r\n terrorism,\r\n or\r\n other\r\n causes\r\n beyond\r\n our\r\n control.\r\n In\r\n such\r\n cases\r\n we\r\n will\r\n make\r\n our\r\n best\r\n effort\r\n to\r\n offer\r\n alternate\r\n dates\r\n for\r\n travel.\r\n BTC\r\n will\r\n not\r\n be\r\n responsible\r\n for\r\n any\r\n additional\r\n costs\r\n associated\r\n with\r\n such\r\n circumstances.\r\n If\r\n Volunteer\r\n withdraws\r\n from\r\n the\r\n trip\r\n due\r\n to\r\n such\r\n causes,\r\n the\r\n same\r\n withdrawal\r\n policy\r\n as\r\n above\r\n will\r\n remain\r\n in\r\n effect.\r\n <\/p>\r\n <h4>Volunteer's\r\n Responsibility\r\n for\r\n Costs,\r\n Third-Party\r\n Suppliers<\/h4>\r\n \r\n <p>\r\n The\r\n trip\r\n fee\r\n covers\r\n all\r\n food,\r\n water,\r\n lodging,\r\n in-country\r\n ground\r\n transportation,\r\n administrative\r\n expenses,\r\n and\r\n program\r\n costs.\r\n Volunteer\r\n is\r\n responsible\r\n for\r\n <\/p>\r\n <p>\r\n <ol>\r\n <li>\r\n Travel\r\n to\r\n and\r\n from\r\n the\r\n starting\r\n point\r\n of\r\n the\r\n trip,\r\n which\r\n begins\r\n at\r\n the\r\n airport\r\n of\r\n your\r\n trip's\r\n country\r\n destination.\r\n <\/li>\r\n <li>\r\n any\r\n additional\r\n costs\r\n incurred\r\n during\r\n free\r\n time,\r\n personal\r\n time,\r\n or\r\n on\r\n activities\r\n which\r\n are\r\n outside\r\n the\r\n trip,\r\n <\/li>\r\n <li>\r\n any\r\n costs\r\n incurred\r\n should\r\n Volunteer\r\n be\r\n injured,\r\n become\r\n sick,\r\n or\r\n otherwise\r\n require\r\n a\r\n doctor\u2019s\r\n visit\r\n or\r\n hospitalization,\r\n or\r\n <\/li>\r\n <li>\r\n withdraw\r\n early\r\n from\r\n or\r\n remain\r\n past\r\n the\r\n time\r\n scheduled\r\n for\r\n the\r\n trip.\r\n <\/li>\r\n <\/ol>\r\n <p>\r\n BTC\r\n is\r\n not\r\n responsible\r\n for\r\n the\r\n acts\r\n or\r\n failures\r\n to\r\n act\r\n of\r\n anyone\r\n who\r\n provides\r\n goods\r\n or\r\n services\r\n in\r\n connection\r\n with\r\n the\r\n trip.\r\n In\r\n addition,\r\n BTC\r\n is\r\n not\r\n responsible\r\n for\r\n any\r\n costs,\r\n losses,\r\n damages,\r\n interest,\r\n or\r\n penalties\r\n that\r\n Volunteer\r\n may\r\n incur\r\n because\r\n of\r\n delays\r\n or\r\n losses\r\n caused\r\n by\r\n any\r\n airline,\r\n other\r\n transportation\r\n or\r\n car\r\n rental\r\n company,\r\n travel\r\n agent,\r\n hotel,\r\n restaurant,\r\n medical\r\n facility\r\n or\r\n other\r\n individual\r\n or\r\n organization\r\n providing\r\n goods\r\n or\r\n services,\r\n regardless\r\n of\r\n who\r\n arranged\r\n or\r\n paid\r\n for\r\n such\r\n goods\r\n or\r\n services,\r\n or\r\n because\r\n of\r\n any\r\n act\r\n or\r\n failure\r\n to\r\n act\r\n by\r\n any\r\n governmental\r\n entity,\r\n including\r\n any\r\n delay\r\n or\r\n change\r\n of\r\n travel\r\n plans\r\n caused\r\n by\r\n any\r\n action\r\n by\r\n any\r\n governmental\r\n entity.\r\n <\/p>\r\n <p>\r\n Dismissal\r\n from\r\n the\r\n Program\r\n BTC\r\n has\r\n the\r\n authority\r\n at\r\n any\r\n time\r\n and\r\n in\r\n its\r\n sole\r\n discretion\r\n to\r\n dismiss\r\n Volunteer\r\n from\r\n the\r\n trip.\r\n Causes\r\n for\r\n dismissal\r\n may\r\n include,\r\n but\r\n are\r\n not\r\n limited\r\n to:\r\n <ol>\r\n <li>\r\n Behavior\r\n that\r\n is\r\n destructive,\r\n abusive,\r\n violent,\r\n or\r\n involves\r\n harassment,\r\n theft,\r\n vandalism,\r\n or\r\n chronic\r\n defiance\r\n of\r\n policies.\r\n <\/li>\r\n <li>\r\n Refusal\r\n to\r\n abide\r\n by\r\n the\r\n laws\r\n and\r\n regulations\r\n of\r\n the\r\n country\r\n and\r\n local\r\n community\r\n or\r\n of\r\n the\r\n United\r\n States\r\n of\r\n America\r\n and\r\n \/or\r\n the\r\n State\r\n of\r\n New\r\n York,\r\n or\r\n to\r\n behave\r\n in\r\n a\r\n manner\r\n that\r\n is\r\n appropriate\r\n therein,\r\n all\r\n as\r\n determined\r\n by\r\n BTC\r\n in\r\n its\r\n sole\r\n discretion.\r\n <\/li>\r\n <li>\r\n Engaging\r\n in\r\n inappropriate\r\n sexual\r\n activity,\r\n unauthorized\r\n absence\r\n from\r\n the\r\n group,\r\n or\r\n failure\r\n to\r\n abide\r\n by\r\n curfews\r\n and\/or\r\n other\r\n set\r\n policies\r\n or\r\n expectations.\r\n <\/li>\r\n <li>\r\n Possession\r\n or\r\n use\r\n of\r\n any\r\n weapons,\r\n non-prescribed\r\n drugs,\r\n or\r\n narcotics.\r\n <\/li>\r\n <\/ol>\r\n <h4>\r\n Deterioration\r\n of\r\n Volunteer's\r\n physical\r\n health\r\n or\r\n mental\r\n condition.\r\n <\/h4>\r\n \r\n <p>\r\n BTC\r\n personnel\r\n may\r\n discuss\r\n Volunteer's\r\n situation,\r\n including\r\n Volunteer's\r\n health,\r\n physical,\r\n or\r\n mental\r\n condition,\r\n with\r\n any\r\n person,\r\n including\r\n other\r\n BTC\r\n personnel,\r\n family\r\n members,\r\n or\r\n medical\r\n personnel,\r\n and\r\n release\r\n any\r\n information\r\n concerning\r\n Volunteer\r\n to\r\n such\r\n person.\r\n If\r\n Volunteer\r\n is\r\n dismissed\r\n from\r\n the\r\n trip,\r\n BTC\r\n will\r\n provide\r\n transport\r\n to\r\n the\r\n appropriate\r\n airport\r\n or\r\n will\r\n arrange\r\n for\r\n other\r\n means\r\n of\r\n departure.\r\n Volunteer\r\n will\r\n be\r\n responsible\r\n for\r\n all\r\n costs\r\n incurred\r\n as\r\n a\r\n result\r\n of\r\n such\r\n dismissal,\r\n and\r\n there\r\n will\r\n be\r\n no\r\n refund\r\n of\r\n all\r\n or\r\n any\r\n portion\r\n of\r\n the\r\n Program\r\n Fee.\r\n <\/p>\r\n \r\n <h4>Personal Property Loss<\/h4>\r\n \r\n \r\n <p>\r\n BTC\r\n takes\r\n all\r\n reasonable\r\n measures\r\n to\r\n ensure\r\n the\r\n security\r\n of\r\n Volunteer's\r\n personal\r\n belongings.\r\n However,\r\n petty\r\n theft\r\n occurs\r\n worldwide.\r\n BTC\r\n will\r\n not\r\n be\r\n held\r\n responsible\r\n for\r\n the\r\n loss\r\n of,\r\n theft\r\n of,\r\n or\r\n damage\r\n to,\r\n Volunteer's\r\n personal\r\n property\r\n or\r\n equipment.\r\n <\/p>\r\n <p>\r\n BTC\r\n strongly\r\n advises\r\n Volunteer\r\n not\r\n to\r\n bring\r\n expensive\r\n or\r\n irreplaceable\r\n items\r\n including,\r\n but\r\n not\r\n limited\r\n to,\r\n cameras,\r\n computers,\r\n portable\r\n music\r\n players,\r\n cell\r\n phones,\r\n other\r\n electronic\r\n equipment,\r\n and\/or\r\n jewelry\r\n on\r\n the\r\n Program.\r\n <\/p>\r\n <h4>Medical Insurance and Emergency Treatment<\/h4>\r\n <p>\r\n It\r\n is\r\n highly\r\n recommended\r\n that\r\n volunteers\r\n purchase\r\n \"Trip\r\n Cancellation\r\n Insurance\"\r\n at\r\n the\r\n same\r\n time\r\n they\r\n register\r\n for\r\n the\r\n trip\r\n to\r\n protect\r\n their\r\n investment.\r\n <\/p>\r\n <p>\r\n BTC\r\n will\r\n provide\r\n emergency\r\n medical\r\n evacuation\r\n insurance\r\n to\r\n all\r\n Volunteers\r\n and\r\n maintain\r\n employer's\r\n liability\r\n insurance\r\n for\r\n any\r\n claims\r\n by\r\n Volunteers\r\n as\r\n part\r\n of\r\n the\r\n trip\r\n fee.\r\n Volunteer\r\n acknowledges\r\n that\r\n all\r\n information\r\n that\r\n Volunteer\r\n has\r\n supplied\r\n to\r\n BTC\r\n concerning\r\n his\r\n or\r\n her\r\n medical\r\n condition\r\n and\r\n physical\r\n and\r\n mental\r\n health\r\n is\r\n accurate\r\n and\r\n complete.\r\n Volunteer\r\n acknowledges\r\n they\r\n have\r\n reviewed\r\n the\r\n emergency\r\n medical\r\n evacuation\r\n insurance\r\n coverage\r\n (provided\r\n upon\r\n request)\r\n and\r\n are\r\n aware\r\n of\r\n the\r\n coverage\r\n limits\r\n of\r\n that\r\n insurance.\r\n Volunteers\r\n should\r\n check\r\n with\r\n their\r\n medical\r\n insurance\r\n carrier\r\n and\r\n be\r\n aware\r\n of\r\n the\r\n limits\r\n of\r\n their\r\n own\r\n medical\r\n coverage\r\n while\r\n traveling\r\n internationally.\r\n Volunteers\r\n should\r\n make\r\n arrangements\r\n to\r\n provide\r\n additional\r\n or\r\n supplemental\r\n insurance\r\n to\r\n provide\r\n for\r\n such\r\n coverage,\r\n in\r\n the\r\n case\r\n where\r\n medical\r\n services\r\n are\r\n required.\r\n <\/p>\r\n <p>\r\n Should\r\n any\r\n medical\r\n emergency\r\n arise\r\n which\r\n precludes\r\n Volunteer's\r\n ability\r\n to\r\n consent\r\n to\r\n emergency\r\n treatment,\r\n BTC\r\n will\r\n endeavor\r\n to\r\n communicate\r\n with\r\n the\r\n person\r\n designated\r\n by\r\n Volunteer\r\n to\r\n request\r\n permission\r\n for\r\n any\r\n necessary\r\n treatment.\r\n If\r\n BTC\r\n personnel\r\n believe,\r\n in\r\n their\r\n sole\r\n discretion,\r\n that\r\n time\r\n or\r\n circumstances\r\n do\r\n not\r\n permit\r\n such\r\n communication,\r\n Volunteer\r\n authorizes\r\n BTC\r\n to\r\n consent\r\n on\r\n their\r\n behalf\r\n to\r\n any\r\n medical\r\n treatment,\r\n including\r\n all\r\n types\r\n of\r\n medical\r\n examinations,\r\n diagnosis,\r\n medication,\r\n or\r\n physician\r\n or\r\n hospital\r\n care,\r\n that\r\n is\r\n deemed\r\n advisable\r\n by,\r\n and\r\n is\r\n to\r\n be\r\n rendered\r\n under\r\n the\r\n general\r\n or\r\n special\r\n supervision\r\n of\r\n any\r\n hospital,\r\n medical\r\n clinic\r\n or\r\n physician.\r\n Volunteer\r\n agrees\r\n not\r\n to\r\n hold\r\n BTC\r\n responsible\r\n for\r\n actions\r\n or\r\n consequences\r\n relating\r\n to\r\n any\r\n such\r\n medical\r\n or\r\n emergency\r\n treatments\r\n or\r\n the\r\n failure\r\n to\r\n provide\r\n such\r\n medical\r\n treatment.\r\n <\/p>\r\n <h4>Miscellaneous<\/h4>\r\n \r\n \r\n <p>\r\n Governing\r\n Law:\r\n This\r\n Agreement\r\n shall\r\n be\r\n governed\r\n by\r\n the\r\n laws\r\n of\r\n the\r\n State\r\n of\r\n New\r\n York\r\n and\r\n any\r\n action\r\n to\r\n enforce,\r\n construe\r\n or\r\n modify\r\n this\r\n Agreement\r\n shall\r\n be\r\n filed\r\n in\r\n the\r\n federal\r\n or\r\n state\r\n courts\r\n in\r\n the\r\n County\r\n of\r\n Westchester\r\n in\r\n the\r\n State\r\n of\r\n New\r\n York.\r\n <\/p>\r\n <p>\r\n Attorney\r\n Fees:\r\n The\r\n non-prevailing\r\n party\r\n will\r\n pay\r\n the\r\n prevailing\r\n party\r\n its\r\n attorneys\u2019\r\n fees\r\n and\r\n costs\r\n in\r\n any\r\n legal\r\n proceeding\r\n brought\r\n by\r\n the\r\n prevailing\r\n party\r\n to\r\n enforce\r\n the\r\n terms\r\n of\r\n this\r\n Agreement.\r\n <\/p>\r\n <p>\r\n Severability:\r\n Each\r\n provision\r\n of\r\n this\r\n Agreement\r\n shall\r\n be\r\n considered\r\n severable.\r\n If\r\n for\r\n any\r\n reason\r\n any\r\n provision\r\n herein\r\n is\r\n determined\r\n to\r\n be\r\n invalid,\r\n such\r\n invalidity\r\n shall\r\n not\r\n impair\r\n or\r\n otherwise\r\n affect\r\n the\r\n validity\r\n of\r\n the\r\n other\r\n provisions\r\n of\r\n this\r\n Agreement.\r\n Moreover,\r\n the\r\n Parties\r\n agree\r\n to\r\n replace\r\n such\r\n invalid\r\n provision\r\n with\r\n a\r\n substitute\r\n provision\r\n that\r\n will\r\n satisfy\r\n the\r\n intent\r\n of\r\n the\r\n Parties.\r\n <\/p>\r\n <p>\r\n Entire\r\n Agreement:\r\n This\r\n Agreement\r\n represents\r\n the\r\n entire\r\n agreement\r\n of\r\n the\r\n Parties\r\n and\r\n may\r\n not\r\n be\r\n changed\r\n except\r\n by\r\n a\r\n written\r\n document\r\n signed\r\n by\r\n both\r\n Parties.\r\n <\/p>\r\n <h4>Use of Volunteer's Likeness<\/h4>\r\n \r\n <p>\r\n Volunteer\r\n consents\r\n to\r\n the\r\n use\r\n of\r\n Volunteer's\r\n likeness\r\n (whether\r\n photographed,\r\n filmed\r\n or\r\n videotaped)\r\n and\/or\r\n oral\r\n or\r\n written\r\n comments\r\n or\r\n any\r\n portion\r\n thereof,\r\n in\r\n any\r\n manner\r\n now\r\n or\r\n in\r\n the\r\n future,\r\n including,\r\n without\r\n limitation,\r\n in\r\n fund-raising\r\n and\r\n promotional\r\n materials,\r\n advertising\r\n for\r\n television,\r\n radio,\r\n print\r\n or\r\n other\r\n media,\r\n and\r\n any\r\n other\r\n BTC\r\n presentations\r\n or\r\n publications.\r\n Volunteer\r\n agrees\r\n that,\r\n in\r\n connection\r\n with\r\n such\r\n use,\r\n BTC,\r\n and\r\n persons\r\n acting\r\n for\r\n or\r\n on\r\n behalf\r\n of\r\n BTC,\r\n may\r\n identify\r\n Volunteer\r\n by\r\n name,\r\n age,\r\n city\r\n and\/or\r\n state\r\n or\r\n country\r\n of\r\n residence.\r\n <\/p>\r\n <h4>Acknowledgment and Release<\/h4>\r\n \r\n \r\n <p>\r\n Volunteer\r\n acknowledges\r\n that\r\n the\r\n trip\r\n involves\r\n travel\r\n to\r\n a\r\n developing\r\n country\r\n with\r\n a\r\n possibly\r\n unstable\r\n political\r\n system\r\n and\r\n differing\r\n cultural\r\n expectations\r\n and\r\n practices.\r\n Volunteer\r\n acknowledges\r\n that\r\n there\r\n are\r\n risks\r\n inherent\r\n in\r\n such\r\n trips,\r\n including,\r\n but\r\n not\r\n limited\r\n to,\r\n forces\r\n of\r\n nature,\r\n accident\r\n or\r\n illness\r\n in\r\n remote\r\n places\r\n without\r\n immediate\r\n access\r\n to\r\n adequate\r\n medical\r\n facilities,\r\n a\r\n lower\r\n standard\r\n of\r\n sanitation,\r\n work\r\n site\r\n injury,\r\n waterborne\r\n infections\r\n and\r\n diseases,\r\n insect\r\n and\r\n animal\r\n bites,\r\n and\r\n travel\r\n by\r\n air,\r\n automobile,\r\n truck,\r\n on\r\n foot\r\n or\r\n by\r\n other\r\n conveyance.\r\n <\/p>\r\n <p>\r\n As\r\n stated\r\n in\r\n the\r\n health\r\n information\r\n section,\r\n Volunteers\r\n must\r\n provide\r\n complete\r\n and\r\n current\r\n health\r\n information,\r\n and\r\n are\r\n strongly\r\n advised\r\n to\r\n consult\r\n a\r\n medical\r\n doctor\r\n for\r\n information\r\n about\r\n immunizations,\r\n your\r\n physical\r\n preparedness\r\n to\r\n undertake\r\n strenuous\r\n work\r\n activities\r\n at\r\n one\r\n of\r\n the\r\n work\r\n sites,\r\n and\r\n any\r\n medications\r\n that\r\n may\r\n be\r\n necessary.\r\n Volunteer\r\n assumes\r\n all\r\n the\r\n risks\r\n of\r\n the\r\n trip,\r\n including\r\n health\r\n and\r\n personal\r\n safety\r\n risks,\r\n and\r\n waives\r\n all\r\n claims\r\n for\r\n loss\r\n or\r\n injury\r\n to\r\n person\r\n or\r\n property\r\n while\r\n participating\r\n in\r\n any\r\n of\r\n the\r\n activities\r\n contemplated\r\n thereby,\r\n whether\r\n such\r\n damage,\r\n loss,\r\n or\r\n injury\r\n results\r\n from\r\n the\r\n negligence\r\n of\r\n BTC\r\n or\r\n its\r\n officers,\r\n directors,\r\n representatives,\r\n employees,\r\n liability\r\n insurance\r\n carriers\r\n or\r\n agents,\r\n or\r\n from\r\n any\r\n other\r\n cause.\r\n <\/p>\r\n <p>\r\n Therefore,\r\n in\r\n consideration\r\n of\r\n the\r\n right\r\n to\r\n participate\r\n in\r\n the\r\n trip,\r\n and\r\n of\r\n the\r\n services,\r\n food\r\n and\r\n transportation\r\n arranged\r\n by\r\n BTC,\r\n and\r\n its\r\n agents\r\n and\r\n associates,\r\n the\r\n undersigned\r\n Volunteer,\r\n intending\r\n to\r\n be\r\n legally\r\n bound,\r\n consents\r\n to\r\n the\r\n assumption\r\n of\r\n such\r\n risks,\r\n and\r\n agrees\r\n to\r\n forever\r\n waive,\r\n discharge\r\n and\r\n release\r\n for\r\n Volunteer\r\n and\r\n Volunteer's\r\n heirs,\r\n executors,\r\n and\r\n administrators,\r\n all\r\n rights\r\n and\r\n claims\r\n for\r\n injuries,\r\n damages,\r\n illness,\r\n losses,\r\n demands\r\n and\r\n other\r\n actions\r\n of\r\n every\r\n kind\r\n and\r\n nature\r\n whatsoever,\r\n which\r\n Volunteer\r\n may\r\n have\r\n against\r\n BTC\r\n or\r\n its\r\n officers,\r\n directors,\r\n employees,\r\n liability\r\n insurance\r\n carriers\r\n and\r\n agents\r\n and\r\n all\r\n of\r\n those\r\n entities'\r\n representatives,\r\n successors\r\n and\r\n assigns,\r\n resulting\r\n from\r\n or\r\n in\r\n connection\r\n with\r\n the\r\n trip\r\n or\r\n any\r\n other\r\n activities\r\n arranged\r\n for\r\n Volunteer\r\n by\r\n BTC\r\n and\/or\r\n its\r\n agents\r\n and\r\n associates.\r\n <\/p>\r\n <p>\r\n <span class=\"mandatory\">* <\/span> <input
